Definitely try before you buy: Sizing can be tricky, especially with the chin bar.  If it's easy to get your head into a Large size, the helmet may sit too loose.  If the Medium is the right size for you, you might consider upgrading your ears to a recessed model (I assume you still have the average-Joe type, mounted externally on the skull).

My Cascade is a bit loose for me, so it can shift pretty far forward, leaving my neck vulnerable.  After getting hit in the base of the skull a couple of times (hurt like an SOB), I added a piece of foam on the back.  Another time, the edge of the helmet hit me on the bridge of the nose (I was face-planting into the left wall of Groove Tube at the moment, so the chin bar worked as designed).

Other than that - I've had it for several years, and it served me well.  Doesn't impare hearing (which is what I don't like about many other models), and doesn't cost too much.

I don't have the premium model with an air bladder - I doubt the bladder will last more than a year though.
