In Atlanta I've gone to Hancock Fabrics to purchase lycra.  It's nice because it stretches and can be formed into some crazy contours.  Try to find bi-directional stretch fabric (stretchy up-down side-side... some only stretch in one direction).

I've used super77 on both fabric and foam to adhere.  If the foam is smooth enough, the fabric covered product can look like it came  from the factory.

Note that lycra can be pretty slippery so make sure the foam is quite snug to keep your booty planted.
