As for labeling yourself, that's not going to help increase your self-confidence, so stop doing it! Don't care about where you are compared to others, but care about where you are compared to where you were before. As for running the "prerequisite" stuff, why care about what others think is needed to run the Green? You're the only one who's able to decide whether or not you want to run something. Maybe you simply don't like class V stuff, and maybe you never will be able to run it. So what?
Keep working on your skills, moving the limit slightly higher every time by running more difficult lines in class IV rapids. I bet that you can come up with more difficult lines, by simply adding one more eddy or move to a rapid that you already can ace on the normal lines. That builds skill and confidence, which helps to deal with fear.  Still, don't hesitate to ask yourself if moving up to class V is what *you* really want, of if it's maybe just pressure from someone else that you feel.
