We're probably all unaware of some secret gnar gem tucked away in the deepest darkest reaches of some sparsely populated mountain range that gets run solo by a buckskin-clad hermit in his home made creature craft.  These kinds of questions are impossible to answer definitively.  Part of the issue is that the most difficult runs in the West require lots of hiking, usually multiple days, and a level of exposure that the East doesn't have.  Conversely, the East has some wickedly technical stuff that may be more difficult to paddle but logistically easier and less exposed.

Hard runs that get run:

Crystal Gorge, CO
Devil's Postpile, CA (not sure if this gets run every year, but it seems to be getting a lot more traffic since 7 Rivers)
Bull Lake Creek, WY
Salmon River Gorge, OR

Honorable Mention:
That 'Prong' or 'Fork' in Leland's book that drops something like 1,000 fpm and was run solo (Garden of the Gods?)
Savage Indian Creek, WV (at least that's what Don likes to call it, probably not done yearly for precipitation reasons)
Upper Sillwater "3 Day Mission", MT
