The individual was in a XXX, got squirted out of a hole, flipped and rolled with their head on the stern. There was swirling water in this area and before the paddler had a chance to sit up, the water had grabbed the stern and flipped the kayaker again. Where was the paddlers face this time while upside down? Facing the bottom of the river i.e.,  against a rock.

I have seen people tweek their shoulders doing the same thing.....lying on the stern where it's most difficult to hip snap effectively then slapping the water with an extended arm trying to right the kayak. When they can't fight it any longer, they flip....arm extended face exposed.

Rather than work on their roll, a lot of people I know who use this style of roll have tried to compensate by installing a face mask on their helmet.
